Subject: Injured keeper walks off, leaving open goal

I was playing a cup game in extra time, having substituted 3 players already. My keeper got injured in minute 101, my team scored quickly taking the lead by one goal. As it was the first break in the game after my keeper's injury, he walked off, leaving my team with an open goal for the remaining 18 minutes. This way, my opponent managed to score 3 goals from just kicking whenever they had some space on my half of the pitch.

This was very frustrating, as this caused me to lose the game I was likely to win (against the odds), as tactics became redundant due to the gaping open goal behind my defenders.

I see this as a bug, as it is impossible this could ever happen in a football game (and I don't think this can be dealt with beforehand using match orders). What I think should happen is, either the keeper should stick out injured, with reduced capacity ofcourse, or the player on the pitch with the highest goalkeeping stat should take the GK's place as to not leave the goal open.

Another thing I noticed was that the offside rule is not implemented correctly, as the rules say you are offside if you are standing behins the second-to-last player of the opposing team. Since my GK was somewhere on the way to the hospital I presume, the attacker with only my last defender behind him when getting a pass should have been flagged, but wasn't. I think this should be corrected as well.

It can, just use the "red card or no player" conditional order from the minute 1

Does that work if you have already done three changes earlier?
I did not think it was possible to change the position of a player on the pitch during a match, but I might be mistaken.
